This tool walks you through a formal 5 Whys root cause investigation, one step at a time.
■ Before you begin
At a minimum, you should be able to describe what happened. The more of the following you bring, the better the tool can advise on what further evidence to gather.
✓
A description of what happenedMinimum
The basic sequence of events as currently understood, plus the date, time, location and type of incident.
The initial incident report or notification
Whatever was first raised when the incident was reported, if one exists.
Accounts from those involved or who saw it
Written statements as files, or verbal accounts you can summarise as context.
Photographs and diagrams
Images of the scene, equipment or conditions, and any drawings or schematics.
Procedures and records
Relevant work instructions, permits, maintenance history or training records, if known.
■ How it works
01
Gather the evidence
Drag in everything you have collected and add the incident details for the report header. You can start with just a description and add more as you go.
02
Answer the guided interview
The tool reads your evidence, then asks one question at a time, adapting each follow up to your answers. It also recommends any further evidence that would strengthen the case.
03
Generate the analysis
When the readiness meter shows you have enough, proceed to a structured 5 Whys report with root causes, corrective actions and ISO 45001 alignment, ready to export to Word.
